The items in the Health Certificate for Short-term Students or for Residence
Applications differ from those required by the Ministry of Education for
students, so it cannot be used as a New Student Health Check Certificate.
Under theregulations, all new students must complete the student health
check (at their own expense) at the beginning of the school term.
1.Please download and print the Student Health Examination Form from the
Division of Sanitary website, and bring it to a medical institution for the
student health check.

2. Alternatively, you can complete the student health check at Fu Jen University
Clinic during service hours (the clinic is located on the 1st floor of Paul
Cardinal Shan Medical Building):

Students with National Health Insurance:
NTD 800 (health check fee) +NTD 50 (dental check registration fee)

Students without National Health Insurance:
NTD 800 (health check fee) +NTD 350 (dental check registration fee)
=NTD 1,150
Notes:
1.Please fast for 8 hours.
2.Female students should avoid their menstrual period for the urine test.
3.Please bring relevant documents(Ex:passport or residence card or student
card).
4.The chest X-ray examination is included, so please wear clothing without
metal.If you are pregnant or suspect pregnancy, please inform us.
5.It takes 5 working days (excluding weekends and national holidays) to
receive the doctor’s consultation on the health check report. On the day of
your return visit, please bring the Student Health Examination Form to
theclinic registration counter for check-in.
